specific details so we can respond accordingly. If
your call is not an emergency, the receptionist may

Appointments
We encourage you to make your appointments as far in advance
as possible. We recognize the importance of your time and
will make every effort to meet your scheduled appointment.

New
Patients
We welcome new patients for consultations or for surgical
treatment. You may contact us directly or through a referring
physician. Please allow 30 minutes of your initial appointment
to complete a medical history form. On your first appointment,
please bring your medical records, and a list of all medicines
you are presently taking.

Cancellations
If you cannot keep a scheduled appointment, please notify
us at least 24 hours in advance. If you must cancel your
appointment due to last-minute unforeseen circumstances,
please let us know immediately.

Billing
and Insurance Information
Our business office representatives are always available
to assist you with any questions you may have. Our business
office telephone number is 281-807-6676. We strongly encourage
you to bring any special needs to our attention. We're here
to help you!
Please bring all pertinent insurance information
and your insurance cards with you when you visit our office.
Medicare, HMOs, and
PPOs
We are Medicare participants and accept assignment for our
services. We also participate in many HMO and PPO plans
as well as insurance plans which permit a free choice of
physicians. Please consult your network directory to ensure
that we are a provider in your plan. As a courtesy, we routinely
file primary insurance claims for all patients. Should you
provide supplemental insurance information to our office,
we will also file those claims on your behalf. The deductible
and insurance co-payments are the patient's responsibility
and their payment is expected at time of service.
Should your insurance carrier withhold payment
or partial payment of your claim for any reason, we will
be glad to assist you in obtaining an explanation from them.
However, we cannot guarantee payment of your claim. Also,
we cannot be responsible for negotiating fees or claims
with insurance companies or any other entity. Patients are
responsible for payment of medical care within a reasonable
time.
If circumstances warrant an extended payment
plan, our insurance representative is available to assist
you with such arrangements. Please contact our office prior
to your appointment.
You will receive an itemized statement showing
all charges. This statement will cover all services provided.
All patients with a current balance will continue to receive
monthly statements until the account is cleared. Any balance
remaining after your insurance company has paid, will be
the responsibility of the patient or supplemental insurance
company, if applicable.
Non-US Residents and
Patients without Insurance
Please contact our business office prior to your visit so
that we can discuss special arrangements regarding your
account.
